Belgium’s Gaming Commission explains how to request a gambling exclusion

Belgium’s Gaming Commission says people can apply for an access ban covering casinos, slot machine arcades and betting, online and offline. Belgian applicants can use an electronic form with an identity card and card reader or the itsme® app; non-Belgians have a separate form, and a signed paper request is also available.
Operators must prevent excluded people from accessing the gambling venues and betting covered by the ban.
